Ho-Ho-Kus Practice
"After transitioning from a large hospital to this smaller, more personal practice, I can now schedule longer visits with my patients and have greater availability when they are sick or have concerns. I'm delighted to bring this level of care to a community I deeply cherish.”
— Karen Hart, MD
“I'm excited to begin at Harley Street Medical, an environment unlike any I've worked in before. I’m thrilled to be able to spend unhurried time with patients and help them navigate an often daunting healthcare system. This is exactly the kind of medicine I've always wanted to practice.”
— Anne Marie Valinoti, MD
Meet the Physicians
Dr. Karen Hart has spent over twenty years practicing internal medicine, most recently as site Medical Director for The Valley Medical Group/Prospect Medical — experience that shaped exactly the kind of care she wanted to build at Harley Street Medical. She has been named a Top Doctor by Castle Connolly every year since 2017.
Her training runs deep: a residency at New York Hospital that included time at Memorial Sloan Kettering Cancer Center and The Hospital for Special Surgery, and an M.D. from Washington University in St. Louis, where she graduated in the Medical Honor Society, AOA, on a four-year full-tuition merit scholarship. Before medicine, she studied electrical engineering at Princeton, then completed post-baccalaureate premedical training at Columbia — a path that gave her an unusually rigorous, analytical foundation for the relationship-based medicine she practices today.
Dr. Anne Marie Valinoti spent more than twenty years practicing internal medicine, including time on faculty at Weill Cornell Medical College and as an attending physician at New York Presbyterian Hospital. Most recently, Dr. Valinoti served as an internist at the Valley Medical Group/Prospect Medical, where she was recognized as a Castle Connolly Top-Doctor for over fifteen years and named an Exceptional Woman in Medicine.
She earned her B.A. in chemistry from Haverford College and her M.D. from Columbia's College of Physicians and Surgeons, completing her residency in Internal Medicine at New York Presbyterian-Weill Cornell Medical Center, with additional training at the Hospital for Special Surgery and Memorial Sloan Kettering Cancer Center. That combination of academic medicine and hands-on hospital training now shapes the more personal, attentive practice she's built in Bergen County.
What is Harley Street Medical?
Harley Street Medical is a concierge internal medicine group with over 40 years of clinical experience. Our physicians are partners, advisors, and advocates for our members, providing hands-on guidance throughout their healthcare journeys.
Our physicians believe in relationship-based, individualized medicine. Smaller patient panels allow for more interactions, longer appointments, and direct physician access for patients, including after hours and on weekends. The all-inclusive membership focuses on preventative care and helping patients better navigate the healthcare system.
What is Concierge Medicine?
Concierge medicine is a model of primary care built around a direct membership relationship between patient and physician, with an annual or monthly fee that supports a higher standard of access, attention, and continuity of care.
Traditional primary care panels typically range from 2,000 to 2,500 patients per physician. Concierge medicine takes a different approach, significantly reducing panel sizes to create room for a deeper, more attentive relationship between physician and patient. There are several models within concierge medicine:
Direct Primary Care (DPC) charges a low monthly fee and focuses on simplifying the payment experience, emphasizing accessibility. It typically involves lighter care coordination and fewer specialist relationships.
Retainer-based concierge practices charge a higher annual fee and reduce panels to roughly 400–600 patients per physician, offering members enhanced access and longer visit times. The depth of care coordination varies by practice.
Ultra-concierge / boutique medicine represents the most comprehensive model — small panels, highly personalized care, and active oversight of a member's full health journey, including specialist coordination and second opinions.
Harley Street Medical operates within this ultra concierge category, with panels of approximately 250 patients per physician — about half the typical concierge benchmark. This allows our physicians to know their patients deeply, serve as dedicated health advocates, and coordinate care across a hand-picked network of specialists throughout New Jersey, New York City, and beyond. Outside of annual exams and regularly scheduled check-ins, all other primary care needs, including laboratory and diagnostic testing, are covered by Harley Street Medical at no additional cost
